Overview

This project focuses on developing an interactive digital tool that empowers patients at risk of falling to understand their challenges and take charge of their recovery. Focusing on both mind and body, the app supports a holistic healing journey by assessing emotional and physical well being. Patients can design personalized mobility plans, access helpful resources, and receive real time support. Encouraging active participation in their recovery both in the hospital and at home.

Goal

A patient centric solution for fall prevention.

Problem

A patient often disregard hospital instructions and ignore to use assistance to move from bed, which results in patient falls.

Hospital staff

Patients often struggle to understand or accept their fall risk, especially when hospital instructions feel generic and impersonal. Once labeled as a fall risk, there is no way to guide them how not to fall or how to deal with risk factors to avoid future falls. while all the protocols are centered towards getting assistance to move.

Patients

While these measures focus on preventing falls, they lack tools that help patients understand their risk, learn how to avoid falls, or improve mobility with confidence. What’s missing is a solution that centers the patient’s voice, needs, and experience.  giving them the tools to navigate their situation, build independence, and actively participate in their own fall prevention journey.

A personalized dashboard that displays all daily tasks assigned and prescribed, that can be scheduled and managed with ease. It also provides a visualization of progress, showing how far users have come with their goals and what tasks are still pending.

Patients can message their assigned nurses with non urgent questions, allowing nurses to reply when available. For calls, patients can check nurse availability and connect only when nurses are marked as available.
