Tailored plans
Tailored plans are designed around your mobility level and stage of recovery, Each plan includes guided activities, clear goals, and progress tracking, with the flexibility to adjust as your needs and abilities change over time.


Goals setting
Library of goals for fall prevention and mobility, with options to select or create custom goal. Tracks progress toward goals, helping patients see their improvements over time and stay motivated.

Emotional Assessment
Mobility assessment enables realistic goal setting and safe activity recommendations tailored to the patient’s abilities.
Physical Assessment
Mobility assessment enables realistic goal setting and safe activity recommendations tailored to the patient’s abilities.
Reports
Progress summery are sent to nurses, supporting effective resource management and personalized care.

Daily Guided Task
Video tutorials
Each task is broken down to actionable steps to follow with videos and audible instructions
Printable materials
Task instructions with images are available as printed handouts for patients who prefer physical copies.
Feed back
Patients are encouraged to provide active feedback on completed tasks and recommend tasks they feel comfortable.

Dynamic dashboard
A personalized dashboard displays all assigned and prescribed daily tasks, making them easy to schedule and manage. It also visualizes progress, helping users see how far they’ve come and what tasks are still pending.

Consultation
Patients can message their assigned nurses with non urgent questions, allowing nurses to reply when available. For calls, patients can check nurse availability and connect only when nurses are marked as available.

Takeaway
Patient education cannot be generic, designing for fall prevention requires acknowledging individual abilities, confidence levels, and recovery journeys.
Empowering patients means listening to their voices and meeting them where they are, rather than forcing one-size-fits-all instructions.
